Pre IVF Investigations

Pre IVF Investigations

IVF treatment involves the fertilization of an egg outside the body. This treatment can be performed either by using the couple’s own eggs and sperm or by using donated sperm and eggs.
  • IVF may be done if your partner has:
  • Fallopian tube damage or blockage
  • Ovulation disorders
  • Premature ovarian failure
  • Endometriosis
  • Uterine fibroids
  • Previous tubal sterilization or removal
  • Impaired sperm production or function.
  • Unexplained infertility.
  • A genetic disorder
Pre IVF Investigation Before commencing the procedure of In Vitro Fertilization (IVF), it is highly important to properly investigate patient’s current situation in order to attain a clear understanding. It will help to decide the most appropriate treatment for the patient. To ensure high chances of conception through IVF, the clinic carries out the following Pre IVF investigations: Ovarian Function Test: Before conducting ovarian function test, proper functioning of ovaries are checked in order to understand the chances of retrieving multiple follicles for the procedure. The use of a series of drugs can be predicted by these tests that may vary from one individual to the other. These tests are based on the current functioning of the ovaries and on the levels of several hormones. Evaluation of the Uterine Cavity: The IVF procedure is aimed at creating zygotes that are placed in the uterus to cause pregnancy. Before carrying out this process, uterine cavity is tested to make sure that it is free from all abnormalities. The common procedures used to determine the health of the uterine cavity are Hysterosalpingogram (HSG) and hysteroscopy. HSG involves passing a dye through the uterus and fallopian tubes and investigating with the help of X-rays. In hysteroscopy, a light and optic scope is used to assess the uterine cavity. These procedures are usually carried out after the end of a menstrual cycle and at the beginning of an ovulation cycle. Testing For Infectious Diseases And Immunity Screening: Blood tests are carried out on both the partners to eliminate any chance of Hepatitis or HIV. Semen Analysis: It is necessary to analyze the semen for the health of the sperms, mobility and count before beginning an IVF cycle. It evaluates certain characteristics of a male’s semen and sperm contained in the body. Depending on the measurement method, characteristics of semen can be evaluated easily. A mock embryo transfer: A mock embryo transfer is a common procedure which is carried out in many clinics including Joy IVF Clinic. An actual embryo is transferred in the middle of IVF cycle, whereas the mock embryo is transferred either before the IVF cycle or during the IVF cycle. This mock embryo is capable of identifying any irregularities in the cervix or the uterus. A special catheter is used to place the embryo into the uterus and then with the help of a sonogram machine, it is ensured whether the uterine cavity is healthy enough to house an embryo or not. Jain Hospital regularly hosts free information seminars/counseling on all aspects of fertility for couples experiencing fertility issues. Fix an appointment with Jain Hospital to avail best treatment.

IUI – (Intrauterine Insemination)

Intrauterine Insemination or Artificial Insemination

In this method of IUI or intrauterine insemination, sperm is introduced into a female’s body to achieve pregnancy through In Vitro Fertilization. This Assisted Reproductive Technology method is safe and effective to treat certain kinds of infertility in both men and women, especially when dealing with natural barriers. This treatment is beneficial for the couples who are suffering from male infertility, single women or those who cannot conceive naturally. In IUI, a semen sample in a nutrient medium is placed in the uterine cavity, that allows sperms to reach uterus. Conditions Warranting IUI
  • For all the single mothers can request a sperm donor that will be artificially inseminated
  • Impotency or premature ejaculation in males
  • For all the males who are unable to contribute naturally for successful fertilization
  • Ovulation disorders such as Poly Cystic Ovary Syndrome
  Preliminary Steps in IUI treatment
  • The ovary of the patient should have healthy eggs
  • The fallopian tubes must be open for a successful IUI treatment
  • The uterus should be normal
  • The semen sample must show an adequate amount of fluid and normal sperms
  • If a sperm donor is used, then, we check for presence of any infectious diseases
Step-wise Procedure in IUI Treatment Ovulation Induction In simple words, ovulation occurs when a mature egg is released from the ovary. It is then pushed down to the fallopian tube and is available to be fertilized. Every month, an egg becomes mature within one of the ovaries and enters the fallopian tube so that it can make its way towards sperm and the uterus. Keeping a Check on IUI treatment Cycle Our team of experienced doctors perform the insemination treatment during ovulation. They use ultrasound scan to determine whether the patient is ovulating or not. Once, it is confirmed that the patient is ovulating, the eggs are released from the follicle with the help of an injection. What is the best timing for IUI Treatment? It is best to perform Intrauterine Insemination treatment during ovulation. Since, washed sperm lasts only for six hours, IUI should be done within the same period but if HCG (Human), a hormone that supports normal development of an egg in ovary is injected then the patients having two IUI will be performed within 24 hours to 48 hours. Semen Collection Semen collection is a process of obtaining semen from males for the purpose of artificial insemination which is collected by masturbation. During this process, doctors instruct the abstinence for  2-4 days before the process. The sample is generally collected at the Jain Hospital itself. Preparing The Semen Sample Next step is the sperm washing which is completed prior to the process. Depending upon the quality of semen sample, methods are adopted by the clinic. The quality of semen depends upon the volume, count and motility. This procedure is conducted with the aim of eliminating all the content other than the motile sperms. Once, the fertilizing capacity of the sample is enhanced well, the sperm sample is used for artificial insemination without much delay.

In-Vitro Fertilization, in which the embryos are created in a test-tube, the babies are thus, called test-tube babies, and the fertilized embryos are implanted in the uterus to begin the process of pregnancy. In IVF, not every embryo implanted leads to pregnancy thus, surplus embryos are frozen – so that a subsequent transfer might be tried if the first one fails. IVF is used to overcome female infertility caused due to problems in fallopian tubes, making natural fertilization difficult. It is also used to assist in male infertility, wherein the sperm cell directly injected to the egg cell, causing pregnancy. With egg donation and IVF, women who have past their reproductive years or menopause can still become pregnant. What is IVF (In vitro Fertilization)? In vitro Fertilization (IVF) Involve putting the eggs and sperm together outside the human body, by natural selection the egg will usually allow only one sperm to enter and this leads to the fertilization process in the laboratory. After fertilization, the embryo are allowed to grow for a short period of time before being placed in to the uterus. A successful pregnancy can be confirmed about 2 week later. What is IVF Treatment ? IVF involves combining eggs and sperm outside the body in a laboratory. Once, an embryo or embryos form, they are then placed in the uterus. What is an IVF Process ? In IVF, the process of fertilization happens outside the woman’s body. A woman’s eggs are surgically removed and fertilized in a laboratory using sperm that has been given as a sperm sample. Next, the fertilized egg, called an embryo, is surgically implanted into the woman’s womb. Typically, one cycle of IVF takes between four and seven weeks. Who can have IVF? Blastocyst Embryo Transfer

Blastocyst Embryo Transfer

What is a Blastocyst?
A blastocyst is an embryo developed at early stages (5 or 6 days) of fertilization in females. A well-developed blastocyst should be able to start hatching from outer shell (zona pellucida) from end of the 6th day of fertilization. The developed embryo should be implanted into the woman’s uterine within 24 hours of hatching. BLASTOCYST EMBRYO TRANSFER The pregnancy through IVF treatment must follow a complete procedure of blastocyst embryo transfer cycle. The laboratory developed embryo should be transferred into the woman’s uterine, when it grew up to 50 to 150 cells. This is called as blstocyst transfer stage, when well developed embryo becomes ready to get transferred into a woman’s womb and grow naturally. In most IVF treatments, successful results have been seen in such type of embryo transfer treatment. A well-developed blastocyst has great possibility to be transferred into the uterine wall than embryos in earlier stages. The blasotosycts which are more developed than early stage embryo have great possibility to grow soon. Advantages and Disadvantages of Blastocysts Stage Advantages: The benefit of growing an embryo up to blastocyst stage is that it allows to select well developed embryo amidst the collection which is then subjected into the uterine wall to grow rapidly. The developed blastocyst increases the feasibility of pregnancy and grow the embryo naturally to get positive results after the cycle is complete. Blastocyst is intended to achieve pregnancy completely and grow at an early stage embryo. It also reduces the chance of miscarriages and increase the chances of pregnancy. Disadvantages: To a major extent, few blastocysts (30 to 50%) have the possibility to survive or grow till this stage. The process required continuous monitoring for checking status of embryo in laboratory which is a big deal for experts. The transfer of blastocyst is a very delicate process that requires much attention and care, if not done, there are chances of miscarriage. Who Is Eligible For Blastocyst Embryo Transfer? This mode of IVF treatment is applicable for those couples, who have failed in conceiving after many attempts. It enhances the chances to achieve pregnancy by growing an early stage embryo till blastocyst stage and then transfer it to the woman’s womb to grow naturally. The embryo will be developed in the IVF laboratory for 5 days after egg retrieval. At least 5 good quality embryos should be available on the third day after egg retrieval to increase the chances of achieving pregnancy. We, at Jain Hospital offer high quality and effective blastocyst embryo transfer solutions for needy patients. Our team of skilled IVF professionals makes it possible to easily transfer the blastocyst developed in our high-tech laboratory and get it transferred into the woman’s womb for further development. We assure our patients to come and experience a safe and result oriented blastocyst transfer services for achieving pregnancy successfully. This can be done at our fully furnished IVF clinic.

Hysteroscopy

Hysteroscopy

Hysteroscopy is a procedure that involves insertion of a narrow telescope-like instrument through the vagina and cervix into the cavity of the uterus (endometrial cavity). The uterine cavity is then distended with fluid and visualized.
What is hysteroscopy used for? This procedure allows us to see if there are any uterine cavity defects such as: ·      Fibroid tumors ·      Endometrial polyps ·      Intrauterine scar tissue ·      A bicornuate uterus or septate uterine malformation ·      Other uterine problems If any defects are found then they should be corrected with operative hysteroscopy which involves placing instruments through ports in the scope that allow us to cut, cauterize, etc. to correct the problem.

Intracytoplasmic Sperm Injection (ICSI)

Intracytoplasmic Sperm Injection (ICSI)

What is ICSI ?
ICSI is the most successful form of treatment for men who are infertile. It differs from In Vitro Fertilization (IVF). In this treatment, embryologists selects a single sperm to inject directly into an egg, whereas in the fertilization process many sperms are placed in a dish near the egg. Male infertility puts bigger challenge as it is much complex than the female infertility process. There are many parameters that need to be looked upon. Now-a-days, there are multiple advance techniques for the treatment of ICSI. At Joy IVF Clinic, we use special tools and equipments to perform activities like picking up the best sperms and inserting them in the egg. Both the activities are very critical, delicate and complicated. The sperms are injected to the core of the eggs and the process requires very high precision and perfection. The success ratio of this treatment is more than 75 to 80 percent. We, at Joy IVF Clinic, use special hormonal injection to the female partner as the uterus needs to be prepared for the pregnancy. Who are the best candidates for ICSI ? There is no specific code of conduct for the use of ICSI. While sometimes, this procedure is used for the treatment of male infertility issues only, in some other cases, it is used for a variety of problems. The use of ICSI has changed in the recent years and a greater number of ICSI cases are seen today as compared to the past. There is no specific code of conduct for the use of ICSI treatment. We, at Jain Hospital, sometimes use ICSI for the treatment of male infertility issues and also for a variety of problems. The use of ICSI has changed in the recent years as a large number of ICSI cases can be seen today as compared to the past. When ICSI is the best alternative? There are some cases when we use  ICSI as the best alternative treatment to solve infertility problem. This treatment can be used-
  • If male partner has very low sperm count
  • Infertility problems with very low concentration of sperms in the semen with very less motility
  • Unsuccessful fertilization through IVF
  • The female partner yield very smaller number of eggs
ICSI: The Modus Operandi We at Jain Hospital hold the matured eggs using a special pipette. These pipette are specially designed to hold the eggs in a better and organized manner. Our team of professionals use sharp and delicate hollow needle to pick up a single sperm in immobilized state. Then the needle with sperm is injected to the shell of the female egg called as Zona. The sperm is injected in the inner part of the eggs called as cytoplasm. This is centre of the egg. The eggs are then monitored next day to ensure normal fertilization.

Laparoscopy

Laparoscopy

This kind of surgical process allows the diagnosis of internal organs. For this, a fabric-optic instrument is used which is inserted into the patients abdomen to view the positioning and working of the internal organs. The major element used for this type of surgery is laparoscope, a fiber-optic cable that is inserted into the human body through the abdomen or chest cavity for internal diagnosis.
Laparoscopy is a modern surgical technique in which human abdomen gets a small incision up to 0.5- 1.5 cm for internal check up of abdomen. This treatment helps to view the working of various internal organs like liver, kidney, uterus, ovaries,  and other pelvic cavities. There are two types of laparoscopy such as: Telescope Rod Lens System This type of laparoscopy includes a video camera (single chip or three chip) in the fabric-optic tube that takes a clear view of internal organs’ working. Digital Laparoscope In this laparoscopy, a charged-coupled device is attached at the end of laparoscope tube that helps in operating the tube to perform the operation inside. Laparoscopy is Helpful in Diagnosis of:
  • Infertility
  • Endometriosis
  • Ovarian cyst
  • Blocked fallopian tubes
  • Pelvic pain
Advantages of Laparoscopy:
  • A complete internal organ diagnosis & treatment
  • Less time consumption in treatment
  • Easy recovery
  • Same day discharge
  • No hassle during operation
  • Less surgical adhesion formation
  • No scars after operation
We, at Jain Hospital offer high quality and secure laparoscopy treatment to patients. Our team of finest doctors and surgeons ensure complete diagnosis of patient’s internal organs by using high quality laparascope tube. We provide easy treatment and relive patients on same day. PESA (Per cutaneous Epididymal Sperm Aspiration)

PESA (Per cutaneous Epididymal Sperm Aspiration)

Being a complex problem, infertility is not only a major concern with females, it can be a drastic situation with males as well. In most infertile cases, males are also responsible for the failure in conceiving. In India, there are many males, who have less sperm counts and they blame their partners or females to be responsible for not conceiving. But exactly, it’s vice versa, there are many males in India, who have low sperm counts and their strength to fertilize the egg for achieving pregnancy naturally. Here, the males are responsible to create the condition of infertility instead of females.
What is Percutaneous Epididymal Sperm Aspiration (PESA)? This is the process through which sperm count of a male can be determined. It is important to check the numbers of sperm and its quality to fertilize egg to get a female pregnant. This is a complex treatment done in laboratory under favorable conditions. For this treatment, doctors collect the sperm of male by introducing needle into the testicles, where the sperms are usually stored. After that the stored sperm will go for laboratory check in terms of its counts and quality. If there is any flaw or a less count in sperm, then the male is recommended to go for a diagnosis for possible modes of IVF treatments. We, at Jain Hospital are pioneers in providing high quality and accurate PESA services for infertile males. Our finest and experienced IVF doctors are instrumental in serving male patients by checking their sperm counts and quality as per their requirement. Our mode of sperm quality check is based on the latest industry norms and are effective to give right results as soon as possible. We test sperms in males at all parameters of quality and counts to find valid reasons of their disabilities or failure in conceiving. We ensure giving accurate and genuine results for checking the quality of sperm and its numbers by possible diagnosis in a high-tech laboratory. So, infertile males facing the issue of less sperm count or any other problem, they can approach our clinic and can experience a complete solution to this problem with positive results at Jain Hospital.

Preimplantation Genetic Diagnosis (PGD)

Preimplantation Genetic Diagnosis (PGD)

Preimplantation Genetic Diagnosis (PGD) is a useful treatment that helps in diagnosing genetic defects or chromosomal deficiency in the embryo created by IVF treatment. This test is required to check quality of an embryo genetically and remove all risks or disorders attached with the embryo during IVF cycle. This is a complete genetic analysis of embryo that tells about the arrangement of chromosomes and their numbers in embryo. However, doctors can identify the flaws and defects in genes transferred in to the embryo created by IVF treatment and remove the problem, if persists on time.
Why PGD Treatment?
  • PGD is necessary for those women who have faced multiple IVF treatments.
  • PGD is required for women having recurrent miscarriages.
  • PGD is for women of old age i. e. more than 38 years.
  • PGD is necessary for women having repeated embryo transfers without achieving pregnancy.
  • PGD is applicable for patients having disturbed genetic and chromosomal numbers during pregnancy.
  • PGD is required for both partners carrying single gene mutations that may affect health of the future child.
  Benefits of PGD Treatment
  • Complete removal of genetic disorders and chromosomal flaws to achieve pregnancy successfully.
  • This treatment benefits pre- implantation of embryo into woman’s womb and help in delivering child genetically same and safe.
  • PGD helps in increasing chances to get a child with no genetic disorders.
  • PGD saves child from any parental genetic flaws and remove them at the first step.
We, at Jain Hospital provide a complete and effective PGD treatment that enable patients to get a genetically safe and healthy child.
